MD Rx Required:

The first step to assist you with a sleep study or provide a cpap machine supplies is we must have a MD RX.Your MD can fax, email,give you the rx signed. We must have this to move forward.

Insurance:

If you plan on using insurance we need a front and back copy of your insurance card.
